Hope Feldman is a Board Certified family nurse practitioner. She enjoys working in a full scope family practice that cares for patients across the lifespan and with a myriad of conditions. In the past decade, she has expanded her scope to provide HIV and Hepatitis C care alongside medication assisted treatment within a primary care model. Having a strong interdisciplinary care team model has strengthened her ability to navigate the needs of her patients with chronic illnesses, especially diabetes.
She has served as a volunteer with ADAs Primary Care Advisory Group for 10 years, most recently in the capacity of vice-chair. She has also been a part of ADAs Professional Practice Committee for 2 years, worked with a joint collaboration with AHA and ACS to synthesize the guidelines, and served as a member of the editorial board for Diabetes Spectrum.
